How do I put together a press kit for my band?

I need to do a press kit to move to the bigger venues. What do I do? I need format. How many parts? How long should it be? What do I talk about? I know about the 5 w's (who what when where and why) but I want to do this exactly right.

You might want to consider contacting the entertainment department of different newspapers and TV stations, and figure out wht they generally require, and tweek it from there.
